Revenue accounting for Medical billings to non-contracted carriers

Looking for resources and/or specific guidance relating to establishing revenue recognition process for medical billings to NON-contracted carriers where net receipt varies (as a % of billing) between carriers and - even within a carrier - CPT codes, in particular when no historical data may exist in new business unit.
