If a plan year begins prior to 12/31/13, can a company obtain another waiver extension?

Our company's health insurance plan expires 9/1/13,  we received a waiver this year for the annual limits.

It looks like we can apply for another waiver at 9/1/13 for one more year. The cost to comply will more than likely result in higher premiums or lower benefits, so we meet that rule.

My question:  because our new plan year begins prior to 1/1/14 (actual date is 9/1/13), can we in fact apply for another waiver extension that will push thru 9/1/14?  If so, where do i locate the extension form to complete?

If not, how are other small companies, more than 50 FTE and less than 1000 FTE dealing with this increased cost?

My understanding from our insurance agent is that you can't get another extension. You have to be compliant even though you start prior to 2014. Check with your insurance company to confirm.
